Health Insurance With GP Access: Every UK Insurer’s Private GP Service Compared

Fast access to a GP is now the most-used benefit of UK private health insurance — but “GP access” means something different at every insurer. Here’s exactly what AXA, Bupa, Vitality, Aviva, WPA and Freedom actually include, the real hours, and what none of it changes about your cover.

Quick Answer

Every major UK health insurer now bundles some form of private GP access — but the detail varies enormously. AXA Doctor at Hand is the only genuinely 24/7 GP video service; Bupa adds 15-minute face-to-face appointments at its own health centres; Vitality GP offers three routes (video, 24/7 phone line and in-person in selected cities); and Aviva includes unlimited digital GP consultations on most plans.

One honest caveat up front: GP access is a service benefit, not extra insurance. It gets you seen and referred faster — it doesn’t change what your policy will actually pay for. Pre-existing exclusions, outpatient limits and pre-authorisation all still apply.

Why GP Access Matters in 2026

Getting a routine NHS GP appointment remains one of the biggest day-to-day frustrations in UK healthcare — and it’s the gateway to everything else, because most private treatment starts with a GP referral. That’s why insurer GP services have quietly become the most-used benefit in private health insurance: members book GP appointments far more often than they ever claim for surgery.

A good insurer GP service gives you same-day or next-day video appointments, private prescriptions, fit notes, and — crucially — a private referral that starts your treatment pathway without waiting for an NHS GP slot. If you’re weighing up whether cover makes sense for you overall, start with is private health insurance worth it? and the wider benefits of private healthcare.

The catch is that “GP access” is marketing shorthand for six quite different services. The table below shows what each insurer actually provides.

Each Service in Detail

AXA Doctor at Hand — the only true 24/7 GP

Powered by Doctor Care Anywhere, AXA Doctor at Hand offers GP appointments by video or phone 24 hours a day, 365 days a year — usually same-day, often within around four hours. GPs are GMC-registered with at least five years’ NHS experience, and Advanced Clinical Practitioners handle daytime demand from 8am to 10pm. Its standout feature: if the GP refers you to a specialist, the referral can go directly to AXA, smoothing pre-authorisation. See also AXA mental health cover and how to claim on AXA.

Bupa — digital GP plus real face-to-face appointments

Bupa’s GP service runs through the My Bupa app (its digital health service is branded “Blua”): video or audio appointments Monday to Friday 6am–10pm and weekends 8am–10pm, plus nurses, physios and mental health specialists. Bupa’s genuine differentiator is 15-minute face-to-face GP appointments at Bupa Health Centres for people on personal policies — something most rivals don’t match. Anyone named on the policy can book, including children, without affecting claims. Compare with the wider Bupa hospital list to judge the whole package.

Vitality GP — three ways to be seen

Vitality GP, delivered by Square Health, gives members video consultations (usually within 48 hours, evenings and Saturdays included), a 24/7 phone advice line, and face-to-face appointments in selected cities such as London, Manchester, Birmingham and Glasgow for a £20 upfront payment. Most core plans include unlimited video consultations, but watch the plan limits — the Essentials plan is capped. Vitality GPs can also refer you onward; check Vitality’s procedure codes and fee schedule before treatment.

Aviva — unlimited digital GP plus second opinions

Aviva includes a Digital GP service with unlimited video consultations per member per policy year on eligible plans, wrapped inside Aviva Smart Health — which also adds a genuinely useful second-medical-opinion service and wellbeing support. A Digital GP referral counts as a valid referral route for Aviva claims, removing the NHS GP wait entirely. For the full picture on the insurer, see our Aviva health insurance review or how it compares in AXA vs Aviva.

WPA and Freedom — check before you assume

WPA takes a modular, medical-first approach — remote GP access depends on the modules you select, so confirm it’s in your build rather than assuming it’s standard. Freedom Health offers virtual GP access that varies by policy tier. Neither is a weakness as such — it’s the trade-off of modular pricing — but it’s exactly the kind of detail a health insurance broker will check for you, free.

Virtual GP vs Face-to-Face GP — What’s the Difference?

Virtual GP (app, video or phone)

  • Included with most major insurers, with extended or round-the-clock hours
  • Ideal for prescriptions, fit notes, referrals and general advice
  • Usually unlimited on core plans (check caps on entry-level tiers)
  • Appointment notes can be shared with your NHS GP

Face-to-face GP

  • Only Bupa and Vitality currently fund in-person GP appointments — Bupa at its own health centres, Vitality in selected cities for £20
  • Better for anything needing physical examination
  • Availability is concentrated in London and major cities

If you’d rather pay for a private GP directly instead of going through insurance, that’s a different (and often sensible) route — see private GP membership costs and our guide to health insurance that covers private GP appointments for how the two approaches compare. Worth knowing: most PMI does not pay for standalone in-person private GP consultations (typically £50–150 self-pay) — the insurer’s own digital GP service is the funded route.

Can GP Access Lead to Faster Specialist Treatment?

Yes — and this is where the benefit earns its keep. An insurer GP can issue a private referral in the same appointment, which means the clock starts on your treatment pathway immediately rather than after an NHS GP wait. From there, depending on your cover, you can be referred for:

Two things to keep straight, though. First, a referral is not authorisation — you (or the GP service) still need pre-authorisation from your insurer before booking treatment, and claims are paid within your policy’s limits. Second, insurer GPs refer privately, not into the NHS — if you want to combine both systems, read mixing NHS and private treatment and jumping the NHS waiting list.

What GP Access Does NOT Change

The honest bit most comparison pages skip

GP access gets you seen faster. It does not widen what your insurance pays for. Three rules still apply in full:

1. New conditions are covered; pre-existing ones aren’t. If you’re already insured and develop a new condition, the GP-to-specialist pathway works beautifully (with referral, pre-authorisation and your excess). But if you buy a policy after symptoms have appeared, that condition will almost certainly be excluded as pre-existing — a GP app doesn’t change that, and self-pay becomes the realistic route. How your history is assessed depends on moratorium vs full medical underwriting.

2. Outpatient limits still cap what happens next. The GP appointment itself is usually unlimited, but the specialist consultations and tests they refer you to draw on your outpatient allowance — check outpatient limits explained and what happens on plans with no outpatient cover.

3. Routine screening generally isn’t funded by PMI. Insurance is for diagnosing and treating new conditions, not preventative checks — see our private cancer screening and men’s health check guides for what screening actually costs self-pay.

What to Check Before Buying

When comparing policies on GP access, the questions that actually separate them are:

  • Included or optional? Standard with AXA, Bupa, Vitality and Aviva on most plans; a module choice with WPA; tier-dependent with Freedom.
  • What are the real hours? Only AXA is 24/7 for GP video — don’t assume the others are.
  • Are consultations unlimited? Usually yes on core plans; entry-level tiers can be capped.
  • Can the GP refer directly, and how smoothly does that feed into pre-authorisation? AXA and Aviva integrate this particularly well.
  • Are your outpatient limits high enough for what the referral leads to?
  • Does it include mental health referral routes, and can family members on the policy use it?

If you’re new to buying cover, our step-by-step guide to how to get private medical insurance walks through the whole process, and buying through a company? GP access is a standard feature of group schemes too — see private healthcare through your business and Aviva business health insurance.

GP Access: Pros & Cons

✔ Pros

  • Same-day or next-day appointments, without NHS GP queues
  • The benefit policyholders genuinely use most often
  • Private prescriptions delivered to your door
  • Direct referral starts your treatment pathway immediately
  • Usually no claims impact and no excess to use it
  • Whole family on the policy can typically book

✘ Cons

  • Mostly video/phone — face-to-face is rare and city-focused
  • Hours vary widely; only AXA is genuinely 24/7
  • Doesn’t expand what your policy pays for
  • Prescriptions are private — you pay for the medication
  • Non-contractual benefit: insurers can change the service
  • Entry-level plans may cap consultations

ℹ Don’t buy on the GP app alone

Every major insurer now has a decent GP service, so it’s rarely the deciding factor. The hospital list, outpatient limits, excess, underwriting and price matter far more over the life of a policy — compare the whole package in our independent insurer reviews, or let a broker do it for you at no cost.

Frequently Asked Questions

Which UK health insurers include GP access as standard?

AXA Health (Doctor at Hand), Bupa (digital GP in the My Bupa app), Vitality (Vitality GP) and Aviva (Digital GP) all include a private GP service on most personal policies. With WPA and Freedom Health it is optional or plan-dependent. Always confirm in your own policy documents, as inclusion can vary by plan and by employer scheme.

Are private GP appointments really available 24/7?

Not always — availability varies more than the marketing suggests. AXA Doctor at Hand offers genuinely 24/7 GP appointments by video or phone. Bupa’s digital GP runs roughly 6am to 10pm on weekdays and 8am to 10pm on weekends and bank holidays. Vitality offers video consultations usually within 48 hours plus a separate 24/7 phone advice line. Check the actual hours for the specific service before you buy.

Can I still use my NHS GP if I have private GP access?

Yes. Private GP access sits alongside your NHS registration and does not affect it. Most insurer GP services can also share your appointment notes with your NHS GP, so your main medical record stays up to date.

Can an online GP refer me straight to a private specialist?

Usually, yes — insurer GPs can issue private referrals, and with AXA the referral can be sent directly to the insurer. But a referral is not the same as authorisation: you still need pre-authorisation from your insurer before treatment, and your outpatient limits, excess and policy exclusions still apply.

Does GP access cover my family too?

Generally, everyone named on the policy can use the GP service, and children are included on most plans — Bupa, for example, lets anyone named on the policy book without affecting claims. Exact eligibility varies by insurer and plan, so check before you rely on it.

Does GP access increase the cost of health insurance?

Where the GP service is included as standard, there is no separate charge — premiums are driven mainly by your age, postcode, excess, outpatient limit, hospital list and underwriting. Where GP access is optional, as with some WPA modules, adding it will affect your premium.

Is GP access worth it if you are healthy?

For many healthy people the GP service is the benefit they actually use, because everyday appointments, prescriptions and quick reassurance are far more common than hospital claims. It should not be the only reason to buy a policy, though — compare the whole package, including hospital list, outpatient limits and price. See is health insurance worth it if you’re healthy?
