Aviva Hospital List Explained 2026: Expert Select, Key, Extended, Trust & Signature
The complete independent guide to all 5 Aviva hospital list options — what each one covers, the real premium impact (verified percentages from broker data), and exactly how to choose the right one for your situation.
In this guide
Aviva offers 5 different hospital list options on its Healthier Solutions policy: Expert Select (guided pathway, around 20% cheaper than Key), Key (around 200 mainstream private hospitals like Spire, Nuffield, Circle, Ramsay), Extended (Key plus London hospitals, around 18% more than Key), Trust (NHS private patient units only — cheapest), and Signature (Scotland and Northern Ireland only). The right choice depends on where you live, whether you want a named consultant, and how much you're willing to pay for premium London access.
What is an Aviva hospital list?
An Aviva hospital list is the network of private hospitals and clinics your policy will cover for inpatient and day-case treatment. Your premium is directly affected by which list you choose — broader lists (especially those including central London facilities) cost more, narrower lists cost less.
Three things determine your Aviva premium more than anything else:
- Hospital list choice — covered in this guide
- Outpatient cover level — see outpatient limits explained
- Excess — the amount you pay before Aviva pays
Hospital list and outpatient cover work together
Even with full hospital list access, if you've reduced outpatient cover, you may still pay out of pocket for consultations and scans. Always consider both together. See: outpatient limits explained.
Aviva's 5 hospital lists at a glance
Here's how the 5 lists compare in 2026, with verified premium impact percentages:
| List | What it covers | London access | Premium impact |
|---|---|---|---|
| Trust | NHS private patient units only | Limited | Cheapest |
| Expert Select | Guided — Aviva picks within 25 miles | Yes (when guided) | ~20% less than Key |
| Signature | Scotland & Northern Ireland only | No | Reduced (regional) |
| Key | ~200 hospitals (Spire, Nuffield, Circle, Ramsay) | Limited | Mid-range |
| Extended | Key + Greater London hospitals | Yes (premium London) | +18% vs Key |
Premium percentages are guide figures
The percentages above are based on broker data for typical 2026 pricing. Your actual quote will depend on your age, location, excess and outpatient choices. Always run like-for-like quotes to confirm exact figures for your situation.
Aviva Expert Select (guided)
Expert Select Most popular
Expert Select is Aviva's guided hospital option. Rather than choosing a fixed list upfront, when you make a claim Aviva shortlists suitable hospitals and consultants within around 25 miles of your home, based on quality criteria. You don't pick the consultant — Aviva does.
This approach allows Aviva to control costs by directing care within their quality-assured network, which is why it's typically around 20% cheaper than the Key list. For most people outside central London, the choice of hospital and consultant Aviva offers will be perfectly suitable.
Best for: cost-conscious members happy to be guided, those outside Central London, anyone who values lower premiums over consultant choice.
Why most outside London should consider this
If you don't need a specific named consultant in central London, Expert Select usually delivers the same quality care at meaningfully lower cost. The trade-off is consultant choice, not care quality. See guided vs consultant choice for the full breakdown.
Aviva Key hospital list
Key Hospital List
The Key list gives you access to around 200 private hospitals across the UK, including most of the major chains: Spire Healthcare, Nuffield Health, Circle Health Group and Ramsay Health Care. With Key, you choose your own consultant — Aviva doesn't guide your selection.
The Key list excludes some of the most expensive central London hospitals like the Wellington Hospital, Cromwell Hospital and HCA London hospitals. If you want those, you need the Extended list. The Key list does include some London facilities, but not the premium ones.
Premium impact: Key sits in the middle of Aviva's pricing — around 20% more than Expert Select but ~18% less than Extended.
Best for: people who want to choose their own consultant, those happy with mainstream private hospitals like Spire and Nuffield, anyone outside central London.
Aviva Extended hospital list
Extended Hospital List
The Extended list is Aviva's most comprehensive option. It includes everything on the Key list plus additional hospitals in Greater London — the premium central London facilities that Key excludes.
Hospitals typically added on the Extended list include The Wellington Hospital, Cromwell Hospital, London Bridge Hospital, Cleveland Clinic London, BMI London Independent and other HCA London hospitals.
The Extended list typically costs around 18% more than the Key list. For people living or working in Zones 1-2 in London, or those who want a specific named consultant who only practises at central London hospitals, this premium can be worth it. For most people outside London, it isn't.
Best for: London residents (Zones 1-2), commuters working in central London, patients wanting access to specific London consultants or premium London facilities.
Do you genuinely need London access?
Many commuters from Surrey, Hertfordshire, Kent and Essex pay for Extended access they barely use. Greater London options on the Key list (or Expert Select's 25-mile radius from home) often cover them adequately. Run like-for-like quotes to see the true cost difference for your situation.
Aviva Trust hospital list
Trust Hospital List
The Trust list is Aviva's lowest-cost hospital option. It restricts treatment to private patient units within NHS Trust and partnership hospitals. These are private wards within NHS hospitals, often offering single rooms and dedicated private patient services, but using NHS facilities and consultants.
The Trust list typically only covers facilities within 30 miles of your home or business postcode. NHS hospitals more than 30 miles away aren't covered.
This is the cheapest list because Aviva pays NHS Trust rates rather than private hospital rates. The trade-off is more limited choice and less of the "hotel-style" comfort of dedicated private hospitals.
Best for: maximum cost saving, people happy to be treated in NHS settings with private patient privacy, those who don't mind less consultant choice.
NHS private patient units — what they actually are
These are private wings within NHS hospitals (sometimes called "Private Patient Units" or PPUs). You get a single room, private nursing, and a named consultant — but the surrounding hospital is NHS-funded. Quality of care is generally excellent (these are major teaching hospitals) but the experience is less "boutique" than a Spire or Nuffield.
Aviva Signature hospital list
Signature Hospital List
The Signature list is only available to Aviva Healthier Solutions members who live in Scotland or Northern Ireland. It offers a reduced premium by excluding all hospitals in England and Wales.
For someone living in Edinburgh, Glasgow, Aberdeen, Dundee or Belfast who has no realistic intention of travelling to England for treatment, this can deliver meaningful premium savings without losing relevant access.
If you live in Scotland or Northern Ireland but might want treatment in England (e.g. specialist consultants in London), Signature isn't for you — you'd need Key or Extended.
Best for: Scotland or Northern Ireland residents who plan to use local hospitals only.
How to check if your hospital is covered
Before buying or claiming, always verify which Aviva list includes the hospital and consultant you want. Aviva publishes an official Hospital List Checker tool you can use to confirm coverage.
- Visit Aviva's Hospital List Checker — search aviva.co.uk for "hospital list checker"
- Enter your postcode — or your business postcode for company schemes
- Filter by list — select Expert Select, Key, Extended, Trust or Signature
- Verify your consultant — confirm they're recognised by Aviva and consult at hospitals on your list
- Always pre-authorise — even when the hospital is on your list, call Aviva before booking. See: how pre-authorisation works
Common London hospitals to check coverage for: London Bridge Hospital, Cromwell Hospital, The Wellington, Cleveland Clinic London, BMI London Independent, The London Clinic.
Which Aviva list suits you?
Six common situations and the hospital list that usually delivers the best balance of cost vs access.
🏡 Family in Surrey/Herts/Kent
If your local hospitals are mainstream private chains, Extended is overkill. Expert Select or Key delivers the same quality at meaningfully lower premium.
🏢 Working in Zone 1-2 London
If you genuinely need flagship central London hospitals or have a named consultant who only practises in W1, Extended is worth the +18%.
🏥 Big city outside London
Manchester, Birmingham, Leeds, Bristol have excellent Spire, Nuffield and Circle hospitals all on the Key list. Extended adds nothing meaningful.
🏴☠️ Scotland or Northern Ireland
If you have no plans to travel to England for treatment, Signature delivers a meaningful premium discount.
💰 Tightest possible budget
NHS private patient units only. Cheapest option, suitable if you're willing to accept slightly less choice and an NHS hospital setting.
👪 Family with children
For routine paediatric care, Expert Select usually covers everything you need. Upgrade to Key if continuity of consultant matters.
How your list interacts with other policy features
Your hospital list doesn't exist in isolation. Three other policy features dramatically affect what your Aviva policy actually covers and costs.
Hospital list + Outpatient cover
Your hospital list determines where you can be treated as an inpatient or day-case. But consultations, scans and most other outpatient activity comes from your outpatient cover allowance, which is separate. You can have full Extended hospital access but still pay out of pocket for consultations if you've reduced outpatient cover. See: outpatient limits explained.
Hospital list + Excess
Your excess (£100, £250, £500, £1,000+ etc.) is the amount you pay before Aviva pays. It applies regardless of which hospital list you choose. Higher excess reduces premium but increases what you pay per claim.
Hospital list + 6-week NHS option
Aviva offers a 6-week NHS wait option that reduces premiums — if the NHS can treat you within 6 weeks, you go NHS instead of private. If they can't, your private cover kicks in. This option can deliver significant savings but means you may end up using NHS rather than your private list. See: 6-week NHS wait option.
Pre-authorisation always required
Even with the broadest list, you must call Aviva to pre-authorise treatment before booking. Hospitals will refuse to bill the insurer without an authorisation code. This is a process step, not optional.
How to choose your Aviva hospital list
1. Map your usage
Where do you live? Where do you work? Are you happy travelling for treatment? Do you have a named consultant who only practises at specific hospitals? Answer these before looking at lists.
2. Use the Aviva Hospital List Checker
Enter your postcode in Aviva's official tool and see which of your preferred hospitals appear on each list. This is the only authoritative source — broker pages and third-party guides can become outdated as Aviva refreshes networks.
3. Run like-for-like quotes
Get quotes for Expert Select, Key, Extended and Trust using identical outpatient and excess settings. The premium difference is often less (or more) than expected, and seeing real numbers makes the decision easier.
4. Decide on freedom vs cost
Expert Select removes consultant choice but saves around 20% vs Key. Trust restricts to NHS settings but is cheapest. Extended gives you everything but costs the most. Your priorities should drive the choice.
5. Speak to an FCA-regulated broker
For complex situations — existing conditions, family cover, switching from another insurer — an FCA-regulated broker can run multi-insurer quotes and explain trade-offs across Aviva, Bupa, AXA, Vitality and WPA. See: best UK health insurance compared.
